It is your responsibility at the onset of treatment to inform Dr. Levine if you intend to use insurance or EAP benefits. If you do, and Dr. Levine is on the provider list for your insurance, please request any required authorization, deductible information (whether annual deductible has already been met) and co-pay amount BEFORE THE FIRST SESSION.

    Dr. Levine is not obligated to accept your insurance if you decide during the course of therapy to change payment options from self-pay to utilizing insurance benefits.

    If you opt to pay out-of-pocket, then seek reimbursement from your healthcare insurance, Dr. Levine can assist you by furnishing a statement that you may submit for reimbursement. Reimbursement from your healthcare insurance will then be between you and the insurance company. This is irrespective of whether Dr. Levine is credentialed with your insurance company or not. Insurance reimbursement will most likely be a portion of the fee paid for services, not the full amount.

    If being utilized, insurance companies will not reimburse for missed sessions. In that case, you will be responsible for the full amount normally due in this case (your usual co-pay, plus the insurance reimbursement rate).
